Este tipo de cirugía se realiza si el paciente no está satisfecho con su voz a pesar del uso de la terapia hormonal masculina, no desea tomar medicación o experimenta efectos secundarios de la misma. A diferencia de la voz femenina, una frecuencia fundamental más baja de la voz se asocia con los hombres.

Antes de la cirugía, se realizará una videolaringostroboscopia precisa y un estudio espectroacústico de la voz para resaltar la frecuencia real y fundamental y planificar la mejor operación para alcanzar los objetivos del paciente.

Una voz masculina es una voz de baja frecuencia, alrededor de 100-125 Hz, que es más baja que la frecuencia promedio de las voces masculinas.

La técnica quirúrgica implica una incisión de 2-3 cm en el área del cuello. Se identifica y reducen los diámetros de la estructura laríngea externa. Las cuerdas vocales se acortan para hacer que la voz sea más profunda y de tono más bajo.

Esta técnica es muy segura y no interfiere con sus cuerdas vocales.

Después de la operación, es posible que sienta disfagia leve, flema en la garganta y bulto.

Debe permanecer en el hospital durante 1-2 noches tras la operación.

Cuidado postoperatorio:

– Día 1 al 7: Debe mantener silencio absoluto hasta la primera consulta de seguimiento. No hay restricciones de alimentos ni bebidas.

– Día 3 al 7: Mantenga las cuerdas vocales lubricadas bebiendo mucha agua y nebulizando con una solución salina esterilizada.

– Día 8 al 10: Después del período de reposo vocal, se realiza un nuevo estudio videolaringostroboscópico y espectroacústico. Se retiran las suturas externas, se cambia el vendaje y puede comenzar la rehabilitación de terapia del habla.

El terapeuta del habla enseñará al paciente cómo usar su nueva voz de la mejor manera posible. El resultado final se verá entre el tercer y el octavo mes después de la cirugía. Por esta razón, se recomienda un seguimiento en el tercer, sexto y duodécimo mes después de la cirugía.

Nota: El equipo quirúrgico se reserva el derecho de evaluar la viabilidad caso por caso si el paciente próximo ya se sometió a una cirugía de cambio de voz utilizando una técnica diferente y no está satisfecho con el resultado de su voz.

Para prevenir el reflujo ácido, es muy recomendable evitar la cafeína, el chocolate, la menta, los alimentos grasos, los alimentos ácidos, los refrescos y las bebidas alcohólicas.

Una semana de descanso laboral es suficiente siempre que sea posible seguir las restricciones vocales. Se requieren dos semanas de descanso laboral si es necesario usar la voz.
