L’isterectomia e ooforectomia sono una procedura chirurgica per ridurre la disforia di genere. Le tecniche utilizzate sono 4 ed esposte come segue:
1. Tecnica laparoscopica:
Invece di praticare una grande incisione, con questa tecnica si eseguono solo alcune piccole incisioni attraverso le quali strumenti sottili passano attraverso l’addome. Questa nuova tecnica riduce significativamente la paura, il dolore, i tempi di guarigione, la perdita di sangue e la durata della degenza rispetto alla procedura in tecnica aperta.

2. Isterectomia addominale con Salpingo-ooforectomia bilaterale (BSO) o tecnica aperta
Viene effettuata un’incisione trasversale nella parete addominale, solitamente sopra l’osso pubico, il più vicino possibile alla linea delle creste iliache dell’individuo, simile all’incisione praticata per un taglio cesareo lunga circa 4-5 cm. Questa tecnica consente al chirurgo un migliore accesso agli organi riproduttivi si fini della rimozione dell’intero complesso riproduttivo. L’isterectomia addominale è il metodo più comune. Il tempo di recupero per un’isterectomia aperta è di 4-6 settimane.

3. Successivamente la tecnica vaginale o V:
L’isterectomia vaginale viene eseguita interamente attraverso il canale vaginale e presenta chiari vantaggi rispetto alla chirurgia addominale, come un minor numero di complicanze, soggiorni ospedalieri più brevi e tempi di guarigione più rapidi. In casi di complicazioni, sarà necessaria l’esplorazione chirurgica.

4. Isterectomia per chirurgia endoscopica transluminale detta “orifizio naturale” con salpingo-ooforectomia bilaterale (BSO) e ooforectomia o tecnica NOTA
Attraverso una nuova tecnica è possibile rimuove l’utero e le ovaie. Questa tecnica deriva dall’isterectomia laparoscopica totale e dalle procedure di isterectomia transvaginale, che offrono maggiore sicurezza. Il chirurgo inserisce lo speculum nella vagina e inserisce al contempo una protezione per proteggere la cervice. Quindi lo strumento chirurgico viene inserito nella vagina. Dopo la dissezione, sia l’utero che le ovaie vengono rimossi attraverso l’apertura della vagina.
